Benefits	Public Service Health Care Plan with Sun Life Optional membership Go to www.pshcp.ca/join/ Complete the Application Form Search on the internet: Pay action request form 446-5E Complete the form correctly Send the forms to the Pay Centre (email address in the second form) Once the Pay Centre has processed your request, finalize the membership by going to www.pshcp.ca/join/positive-enrolment/ Public Service Dental Care with Canada vie Mandatory membership Login to MyGCPay Go to the benefits section and select "See all" Note your certificate number which begins with CF Go to my.canadalife.com/sign-in Select "Register" and click on "Select" Benefits Enter the plan number (055555 ou 055666) and your member number (found in step 3)
